ID Cards

Youth Card

  • Under 18 years of age
  • Expires after 5 years

Adult Card

$5.00 One-time fee
  • At least 18 years of age
  • Never expires

All players, regardless of age, are required to have CYC ID cards.

All coaches must have a CYC ID card, also. Coaches (who are 18 years of age and over/out of high school) need a Coach’s ID number which can be found on the coach’s certificate which you obtain once you have completed the CYC Coach’s Training program.

  • After your player or coach card order has been approved, you will receive an email stating that your card is “in process.”
  • When you place your order, the email with your receipt also has a link to your temporary waiver.
  • Please be sure to print out the waiver. This waiver acts as a substitute for the ID card and is good for three weeks from the date you placed your order.

Our staff is committed to work as quickly and efficiently as possible to approve, print, and mail out CYC cards.  If you have not received your card within two weeks of placing your order, please contact the CYC Office at (314) 792-7256.

I.D. Card Requirements

Players

  • All Players, Pre-K through Adult, must have a CYC Photo ID Card.

Coaches

  • All Coaches of all age groups must have a CYC Photo ID Card. In addition, all Coaches, both Head and Assistant, and all Managers for Kindergarten thru 8th grade (who are 18 years of age and over/out of high school) must have taken the CYC Coaches Training online program or attended a past in-person session.
  • After completing the CYC Coaches Training program, Coaches will then need to apply for their CYC ID Card.
  • After verification from the Safe Environment Program that they are in compliance with the Archdiocesan requirements, adult coaches will then be issued a CYC Coach’s picture ID Card with their Coaches ID number on it.
  • Head Coaches/Managers for High School and Adult Divisions do not need to take the CYC Sports Coaches Training Program, but they do need a CYC Photo ID Card.
  • Coaches who are under 18 years of age must read and sign the Minor’s Code of Ethical Conduct and return it to their parish office. They can use their CYC Youth ID Card, but the roster must reflect that they are a youth coach. They do not need a Coaching ID number.

What to bring to a live ID Card session
Because this is a photo ID, the applicant must be present to have his or her picture taken. Live ID sessions are open to anyone. Each applicant must bring one of the following for verification of birth date. (The item must have the applicant’s correct name and birth date on it):

  • Birth Certificate
  • Baptismal Certificate (must have Birth Date indicated)
  • Current or Expired CYC Photo ID Card
  • Passport
  • Valid Drivers’ License
  • There is a $5 processing fee for each ID card.

Notes about Live ID Card Sessions:

  • Please do not arrive before the published starting times. Registration will not start early.
  • Facilities are in use until the published starting times.
  • Additional (duplicate) CYC Photo ID cards may be purchased at the time of registration.
